Guide lines to using the above Mail Order form.

1.Click on form, then Print off a copy.

2. Select the product/s from the products pages.

3. Fill in details using block capital letters (the code is in brackets).

4. Use the Colour option 2 column as a second choice colour option. Leave blank if colour option 1 is only required.

5. Add up your order total with the P+P charge. Put this figure in the Grand Total box.

6. Sign and print your name, then date and fill in delivery details.

7.With payment (Chqs made out to 'Black Salamander') send form to: Black Salamander, P O Box 107, Neath, SA10 6WZ.

8. Please allow 7 to 10 days to receive your order back.
